About TrueVita Senior Care Solutions
Natalie Murphy is a Registered Social Worker (RSW) and the founder of TrueVita Senior Care Solutions. She is deeply committed to helping older adults and their families navigate the challenges of aging with dignity, confidence, and the right support in place.

Her passion for senior care was shaped by personal experience. Growing up with aging grandparents, she witnessed the emotional and logistical struggles families face when trying to find appropriate care and maintain independence. This early insight led her to pursue a career focused on aging, care transitions, and advocacy.

Natalie holds an Honours Bachelor of Social Work degree from Ryerson University (now Toronto Metropolitan University) and has completed the Geriatric Certificate Program through McMaster University in collaboration with RGP Central, the Regional Geriatric Program of Ontario. Through this specialized training, she is also qualified to administer cognitive screening tools, including the MMSE (Mini-Mental State Exam) and the MoCA (Montreal Cognitive Assessment), to help support early identification of cognitive changes.

Professionally, Natalie brings over a decade of experience working with seniors and their families in both hospital and community care settings.
She began her career at Hamilton Health Sciences, where she worked as a Social Worker supporting patients and families through hospital stays, discharge planning, and care transitions. She later moved into the role of discharge specialist, overseeing discharges across multiple hospital units and providing leadership to a team of social workers. In that role, she focused on ensuring that discharge processes were safe, timely, and person-centred, often navigating complex situations and helping remove barriers to community reintegration.

She later joined Ontario Health at Home, coordinating in-home services and community supports for seniors. During this time, Natalie also completed capacity assessments related to shelter decisions as part of the long-term care (LTC) application process, guiding families through difficult decisions with clarity and compassion.

These roles gave her firsthand insight into the barriers families often encounter, and the critical need for guidance, advocacy, and personalized care planning.

Natalie founded TrueVita Senior Care Solutions to fill that gap. With her extensive experience and deep-rooted passion for elder care, she is dedicated to helping families navigate the aging journey with clarity, compassion, and expert support
